Honolulu FAA failed to report 'close call' to higher ups

"A Hawaii News Now investigation revealed that a mistake, made in January of this year by an air-traffic controller, resulting in two jets being put on a collision course as they approached Honolulu International Airport, was never reported to higher ups at the Federal Aviation Administration."
