Crime Data Investigation
The initial story in the crime data investigation found that from 2009 to early 2012 the Milwaukee Police Department misreported more than 500 aggravated assaults as lesser offenses not counted in the city’s violent crime tally. More than 800 additional cases followed the same pattern but couldn’t be verified with available records. Subsequent stories found police underreported aggravated assaults even when their own officers were severely injured; police clerks routinely changed dangerous weapon codes to generic ones in a way that allowed violent assaults to be underreported — and escape FBI scrutiny; the FBI’s crime auditing process is a fig leaf — metro police departments are rarely audited, and even then the sample sizes are too small to draw meaningful conclusions; Milwaukee police knew they misreported rapes and robberies, but didn’t mention this to city leaders or the public; high-ranking department officials raised red flags internally for years that there were problems but the public only heard a drumbeat that crime was down. In addition to these major installments, Poston and Diedrich wrote nearly two dozen follow-up stories that documented the fallout.

Hospital at Risk
My investigation of the Minnesota Security Hospital, a state-run facility that provides psychiatric treatment to nearly 400 adults deemed "mentally ill and dangerous," uncovered high rates of violence and injuries of employees and patients at the facility, a critical shortage of psychiatrists, and widespread confusion among employees about what to do when a patient becomes violent. I found that much of confusion was the result of the abrasive, threatening management style of head administrator David Proffitt, who was hired in 2011 to reform the facility. I began investigating Proffitt and found he was hired without a basic background check. I uncovered many troubling details from Proffitt's past, including domestic violence, a PhD from a now-defunct online degree mill, a forced resignation from his previous job as the administrator of a private psychiatric hospital in Maine, and other failings. The state ordered Proffitt to resign and the Minnesota legislative auditor began an audit of the department's hiring practices. The assistant commissioner of the Department of Human Services who led the hiring search also resigned. The governor proposed $40 million in renovations to address safety concerns. Regulators from the Occupational Safety and Health Administration visited the facility for the first time in 21 years. The facility also implemented new training for employees to reduce violence. My investigation of the facility continues.

D.C. Tax Office Scandal
The District of Columbia struck an unprecedented number of deals behind closed doors this year with prominent commercial property owners who had appealed their tax assessments, reducing the city's tax base by $2.6 billion. The settlements were kept from the public for months until The Washington Post started mining public records and filing FOIAs, which the city routinely denied until the newspaper's lawyers got involved. The Post also learned that city leaders had kept critical internal audits about the tax office in "draft" format to prevent their release under FOIA. Through sources, The Post obtained the undisclosed reports -- along with a dozen other audits that had been kept from public view -- and published the findings for the first time. The series prompted the City Council to change the law to require the tax office to immediately make public all of its reports -- bringing a new level of transparency to a once secretive agency. The Securities and Exchange Commission also launched a probe to see if the city had kept critical findings from audits used to determine bond ratings. The inquiry is ongoing.

Investigating the IRS
As the national deficit soared, WTHR exposed fraud, confusion and government mismanagement that resulted in illegal immigrants getting billions of dollars in improper tax credits and refunds from the Internal Revenue Service. WTHR gained unparalleled access to tax records and immigrant communities to show exactly how the fraud was committed. The investigation revealed the IRS had known about the widespread problems for a decade but failed to act, and that IRS managers actively encouraged their tax examiners to ignore blatant signs of fraud. WTHR’s investigation quickly gained national attention, attracted more than 9 million online views, sparked intense debate and action by Congress, and triggered immediate reforms by the IRS. Following a series of in-depth follow-ups by WTHR and an Inspector General audit that confirmed all of WTHR’s findings, the IRS announced final rule changes in December designed to reduce the massive fraud and to save taxpayers billions of dollars.

Judge Sylvia James: Access Denied, Chief Judge Under fire
This series examines a chief judge who appears to be using her power and position to do as she pleases. WXYZ used FOIA extensively in their investigation which lead the city to put her on administrative leave as well as leading to inquiries and audits from the State Court Administrator, the Judicial Tenure Commission and a formal complaint containing 192 specific allegations.

Transportation Center Stalled at South Carolina State University
The series investigates where the $50 million in state and federal dollars went that had been given to build a new transportation center at South Carolina State University. With a vacant building site and no underway, school officials did not have an answer as to where the money went. The story prompted lawmakers to launch a formal investigation.

Ghost Tickets
For decades now city officials, city police, and their spouses and friends are not paying their parking tickets and getting away with it. In this investigation of this secret parking ticket favors, it reveals that “millions of dollars” are lost from these parking tickets and the city must find a way to fix the problem. Further, the city’s government is being ridiculed for their lack of control and supervision over such practices.

"Charity Starts at Top"
Certain charities have been paying their executives "excessive salaries" despite federal laws designed to prohibit such an act, the Observer reports. Loosely enforced rules and a lack of staffing in the IRS office that audits nonprofits allows many of the large payouts to go unnoticed.

Code 3
"Code 3" focused on ambulance delays in San Francisco and provided a rare glimpse inside an inherently complex and often secretive bureaucracy. The project began as a two-day series and continued with several follow-up reports. Paramedics and quality control experts say the city does not have enough ambulances and needs to hire more paramedics. A history of tensions between paramedics and firefighters, and a lack of coordination between the Fire Department, the Department of Emergency Management and the Public Health Department, continues to undercut the city's 911 medical responses and the quality of care. The city does not collect sufficient data on 911 responses to fully audit ambulance delays, examine particular treatments and learn from clinical mistakes
