Uncounted Casualties
A three-day series that analyzed causes of death for 266 Texas veterans of Iraq and Afghanistan. The six-month investigation uncovered previously unknown information, pulling data from a variety of federal, state and local sources. The series, which also depended on extensive interviews with family members and fellow service members, revealed the startling number of Texas veterans dying of prescription drug overdoses, suicides and motor vehicle crashes. The newspaper's analysis was hailed by epidemiologists and former Department of Veterans Affairs researchers as an important step in understanding veteran mortality, and led to calls for better government tracking of how veterans are dying.

NC Auto Inspection's-Failing the Test
Every year, North Carolina auto owners must take vehicles to private garages for state-mandated safety and emissions testing meant to prevent traffic crashes and curb pollution. Drivers cannot put a car on the road legally unless it passes inspection. A review inspection data showed the program is undermined by unscrupulous garages who do a volume business, passing unsafe cars, and by other who take bribes or cheat customers with uncessary repairs.

School Buses Flunk
The safety inspections of 30,000 Connecticut school buses showed that one in four buses was unfit to carry children, yet state officials ignored this issue.

State Autos
The Arkansas Democrat-Gazette investigated the number, taxability, and use of state vehicles by state officials. The reporters found that officials weren't paying income taxes on the personal use of their state vehicles and that the state had not been keeping track of the number of state vehicles.

"A License To Lose"
This investigative report reveals weaknesses in security in the Ohio Bureau of Motor Vehicles, as well as in the BMVs of two nearby states. WBNS-TV found that hundreds of social security and registration papers were being discarded in unsecured trash receptacles. The report also reveals that the BMV was aware of the security breach two years prior to the occurrence, but failed to do anything about the issue.

Renegade Riders
Despite new state laws, extra enforcement and self-policing, off-road vehicles are tearing up public lands across the state. As the Minnesota Department of Natural Resources designated nearly 8,000 miles of motorized trails, top officials repeatedly ignored staff experts' recommendations on how to keep riders away from sensitive areas such as wetlands. A companion video reports on startribune.com used a hidden camera to capture illegal off-trail damage as it happened.

Bridge Tracker
After 13 people died in August 2007 when a freeway bridge fell into the Mississippi River in Minneapolis, governors across the nation rushed to calm fears. Using almost identical language, states assured the public that bridges are safe, because federal regulations require inspection of "every bridge at least once every two years." In fact, at lest 17,000 bridges in the U.S. went more than two years between safety inspections, according to federal records obtained by msnbc.com. Obtaining new records from the National Bridge Inventory under the Freedom of Information Act, bill Dedman of msnbc.com gave the public a look at inspection records through 2006. The series of articles documented several lapses in state and federal oversight bridge inspections. The interactive staff at msnbc.com created the Bridge Tracker, an interactive map of bridges, allowing readers to look at the inspection information for bridges they cross. The map shows the condition and inspection dates for more than 100,000 bridges with traffic of at least 10,000 vehicles a day.

Ohio Attorney General: Price of Corruption
WBNS-TV (Columbus, Ohio) revealed a pattern of corruption inside the state's highest law enforcement office including cronyism, misuse of state funds and property, improper use of campaign funds, ethics violations and cover-up. The reporters found that the Attorney General had used campaign funds to rent a condominium for two of his friends/employees that was later tied to sexual harassment,alleged crimes involving state vehicles and the hub for cronyism. Their reporting revealed that the Attorney General created a "transition fund" as an unregulated 501 c4 non-profit account. Through law enforcement, the station learned that this fund funneled at least $2,000 in inappropriate payments to the Attorney General's friend/employee/condo-mate.
