Health magazine investigates whether Dr. Elvin Gaines was more concerned about paying HMO rates than Joyce Ching's medical history. Ching died after doctors on contract to a new kind of HMO failed to diagnose colon cancer in time. The kind of HMO plan Ching was under, so-called capitated plan, made it more profitable when the doctor did less. (May/June 1996)
