Milwaukee Journal finds officials of a company building a sewer tunnel beneath a street had been warned--prior to a fatal blast caused by methane--that they would encounter methane along the tunnel route; an inadequate ventilation system violated federal safety regulations and allowed methane gas to accumulate to explosive levels in the tunnel, January - March 1989.
