WBNS's investigation found children inside a psychiatric facility for teens were repeatedly subjected to incidents of violence and sexual abuse. As part of our investigation, we reviewed more than 400 calls for police service dating back to 2017. We also reviewed 911 calls, children services records, lawsuits and juvenile court records. Through that research, we learned that state and county agencies were aware of the problems but failed to hold the facility to account beyond just requiring it to submit corrective actions plans after each incident. The problems were systemic and have continued unabated for years.
